An anal fistula refers to an aberrant connection between the skin near the anus and the lining of the interior of the anal canal. Anal fistulas are commonly caused by a collection of pus, called an abscess, inside the anal canal. The pus can drain onto the skin on its own or with treatment. The fistula is caused when the pus does not heal properly. This leads to the creation of a narrow tunnel with its external opening in the skin near the anus and an internal opening within the anal canal. If the fistula reaches above the sphincter muscles, the patient may also have to undergo a temporary colostomy, but this is rare.
Treatment
Most anal fistulas require surgery since they cannot heal on their own. Non-surgical treatments may alleviate symptoms but are not permanent solutions.
Anal Fistula Surgery
Anal fistula surgery can take several forms, depending on the location of the fistula:
If the fistula is below or crosses the lower part of the sphincter muscles, your surgeon will cut the fistula open to the skin and leave the wound open so that it can heal with healthy tissue
If the fistula has branches that pass through the upper part of the sphincter muscles, your surgeon may place a special stitch in the fistula to allow plus to drain easily
The fistula may be suitable for treatment with plug made from pig-bowel tissue. Your surgeon may not need to make a cut in the sphincter muscles
Anal fistula surgery usually takes between fifteen to thirty minutes and is performed under general anesthesia.
If bowel incontinence (involuntary bowel movement) is an issue, the patient may need to go through several operations over a period of months to reduce the risk.
Complications
Normal complications of anal fistula surgery are pain, bleeding, and scarring. More significant complications may be difficulty passing urine, involuntary passing wind or lose motion. Bowel incontinence may be a major factor.
Recovery
Patients may be discharged on the same day or remain under observation for 24 hours. After going home, rest is prescribed for the first few days. You should walk as little as possible. The wound takes several weeks to heal completely and patients may need to wear pads till then.

A haemorrhoidectomy is a surgery to treat hemorrhoids or piles. Piles are soft, fleshy lumps that form inside just inside the anus. They easily bleed during bowel movements. Though they are not usually painful, they can cause itching. If the lumps grow, they can protrude outside through the anus in a condition called prolapsed pile. You can feel the lump while cleaning yourself. Hemorrhoids generally develop gradually, over a lengthy duration of time and are often related to constipation. Sometimes, the condition is genetic and made worse by pregnancy.
Treatment
Hemorrhoids can often be dealt with by eating more fiber and more intake of fluid. If these measures do not work, they may be treated with ‘banding’ or dissolving the lumps with injections. If these methods do not work, patients will need to undergo a haemorrhoidectomy to surgically remove the piles.
A haemorrhoidectomy is usually performed under general anesthesia and takes about twenty minutes. The surgeon will usually just cut away the hemorrhoids or use the staple gun technique, which does not create an open wound
Complications
A haemorrhoidectomy may cause the usual complications like pain, bleeding, infection of the surgical wound or blood clots. More serious complications may include the following:
Incomplete haemorrhoidectomy
Difficulty passing urine
Anal stenosis
Developing skin tags
Developing an anal fissure
Incontinence
